Home Birth "Common Ground"?

It may be less well-known than the effort to find common ground between the pro-choice and anti-choice movements but it's no less controversial, it seems.
The growing childbirth advocacy movement has highlighted the divide between those who would like to see expanded access to safe, state-regulated out-of-hospital birth and midwifery for women and those who oppose access to these options.
However, a new effort, nearly three years in the making, to bridge the divide is underway. According to the American College of Nurse-Midwives, a Home Birth Consensus Summit is being convened in 2011 and will bring both camps to the table in an effort to find areas of agreement with the ultimate goal of improving maternity care for pregnant, birthing and postpartum women and their babies.
In a statement about the summit, ACNM notes that the meeting will be facilitated by "the Future Search Network, a nonprofit organization that is internationally known for brokering lasting agreements and shared initiatives in highly volatile and polarized settings, around issues related to poverty, health care access, regional and ethnic conflict, and education. Future Search meetings produce a "Common Ground Agenda," which articulates a shared vision and direction."
It's likely not shocking that this issue is considered "highly volatile," if you're a regular reader of RH Reality Check or have ever been a part of the maternity care system as patient or provider in this country. Despite a steady stream of peer-reviewed, published, scientific studies on the safety of planned home birth and the widespread use of midwives in other countries as credible maternity care providers, mainstream medical associations like the American Medical Association (AMA) and the American College of Obstetrics and Gynecologists (ACOG) are fiercely opposed to any home birth options - licensed and regulated or not. Whereas a host of public health associations, nurses groups, nurse-midwifery organizations, certified professional midwifery advocates and even many individual MDs are working hard, daily, to pass state laws legalizing home birth and certified professional midwifery.
Progress is happening, though. ACOG recently revised its official position on hospital policies regarding the availability of VBACs (vaginal birth after cesarean section), after an immense grassroots advocacy effort by birth advocates and an agreement about the safety of VBACs from an NIH Consensus Development Conference on the issue. The issue is central to home birth access since many women interested in out-of-hospital birth desire less - or no - unnecessary medical interventions and most hospitals have banned even allowing a woman to attempt a vaginal birth, after having had a cesarean section for previous births.
The Home Birth Consensus Summit is hopefully the next step in this journey towards progress. That there is funding for it at all, is the first step in consensus building. The Transforming Birth Fund awarded a grant for the summit to an impressively diverse array of groups to convene the conference in 2011. According to ACNM, the grant proposal was spearheaded by ACNM's Home Birth Section Chair, Saraswathi Vedam, and includes the Midwives Alliance of North American (MANA), the American College of Obstetrics and Gynecology (ACOG), the American Pediatric Association (APA), Association of Women's Health, Obstetric and Neonatal Nurses (AWHON), Lamaze and more.
Home birth and certified professional midwifery are topics lighting some serious fires around the web, amongst not only mothers and providers, but seemingly anyone with an opinion on our health care system right now.
Hop on over to the New York Times Parenting blog, Motherlode, and you'll find a recent post (and polarized discussions in the commenting section) on the fiery debate that's been happening on yet another parenting site, Babble.com, over home birth access. As with abortion access, there is a contigent of those who do not want women to be allowed to access care. In this case, licensed, regulated home birth with a certified professional midwife - despite the fact that 27 states already have deemed this legal and safe. The discussion seems to devolve into whether or not women should trust doctors, whether or not women should automatically defer to hospital policies when it comes to hospital birth (ie, if the policy is to strap a fetal heartrate monitor on every woman, then it must mean it's the right thing to do so don't question it) , and why we can't just put 'newborn lives ahead of our own desire for a certain type of birth.'
But these claims seriously overlook the central issues for home birth and CPM advocates. Desiring and advocating for planned home birth options with a CPM is precisely about newborn health and the experience babies have during birth. Or, at least it is for many women. There are women, with healthy pregnancies, who desire a birth experience that is not automatically medical in nature because they know that evidence - medical evidence - points to the detriment unnecessary medical interventions may (though don't always) impose upon a newborn's health both in-utero and once he or she is born. As well, some women do see birthing at home, with a midwife, as an optimal birth experience - far from the bright lights and antiseptic environment some hospitals (though not all) provide. For women who wish to plan a birth at home, or at a birthing center, it's not about a distrust of physicians so much as it is about a choice not to make it about physician-oriented care at all. Midwives are not medical doctors but they are care providers and care givers.
Motherlode writer Lisa Belkin writes,

Isn’t the obvious answer somewhere in the middle — a recognition that birth should be treated as a natural event, until it isn’t. And that it should happen in a homelike environment within a hospital, because when things go wrong they go wrong quickly?
However, finding common ground when it comes to maternity care isn't that simple. Women in the US are more likely to die of complications resulting from pregnancy or childbirth than women in 49 other countries, including South Korea, Kuwait, and Bulgaria, yet we spend more on health care than any other country in the world, according to Cristina Finch writing on RH Reality Check. Simply creating a "homelike environment" in hospitals isn't what will save women's health and lives. We need to find the common ground that all women can stand on - women who desire a planned home birth, with a licensed, regulated professional midwife; women who desire to birth at a birthing center, without intrusive, unnecesseary medical interventions; and women who need the medical care provided by physicians and surgeons should their birth experience require it; finally, we need to create a common ground that respects the knowledge of both midwife and physician, the distinct models of care which both provide, and the desires and knowledge of our pregnant and birthing mothers who have a right to care which is, by state regulation and evidence-based research, deemed safe and legal. The Home Birth Consensus Summit is banking on finding that common ground, to save the health and lives of our mothers and babies.

The countdown for the 112th Congress to convene is 3 days away. In listening to the rhetoric on the airwaves, it appears the country is poised to become a conservative orderly society for the wealthy, the business community and the anti-choice forces.  The Republicans are threatening to enforce their mandate, which they proclaim is to roll back "Obama Care," cut spending and read the Constitution on the first day in session.

 On January 2nd on Daily Kos,  Kaili Joy Gray outlines the challenges that women will face in the upcoming year.  The right of a woman to control her body is in peril.  In state legislatures across the county there are so many draconian laws concerning abortion that in many areas women will have to travel for abortions or resort to self-performed abortion.  In states like Nebraska, lawmakers have become fetal experts who can determine when and if a fetus feels pain. They must have all had Freudian therapy where they traced their issues back to the womb and were able to identify the pain they felt.  The National Right to Life committee will encourage other states to pass this law as well as the Oklahoma law that mandates a woman pay for an ultrasound before she is allowed to exercise her legal right to abortion.  In Ohio, as well as many other states, legislation to prohibit health insurance coverage for abortion if it is subsidized by state or federal funds will be a weapon to fight against abortion.  Once the new health care law is fully in place in 2014, the anti-choice legislators are plotting to turn the law against the right to choos by preventing women from buying health insurance that covers abortion services.  This will primarily affect women who cannot afford abortions or child care. 

On the federal level, Representative Chris Smith (R-NJ) has longed tried to introduce restrictive abortion laws.  He is now proposing to use the new health care reform law to deny reproductive health care to women.  He is trying to use the power of the purse to subvert the law of the land, Roe vs Wade, by making it financially difficult for women and their employers to pay for health insurance coverage of abortion.

 In 1973, the Supreme Court issued a decision with a 7 to 2 majority in favor of Roe.  This decision was partially based on the 14th Amendment's right to privacy.  This means that abortion is a fundamental right under the Constitution.  So now we have a Supreme Court decision in favor of abortion based on the Constitution.  Where are all the Tea Party conservatives who have said that laws must be based on the Constitution?  Now, we have such a law and these activists are trying to go against the Constitution.  Where are John Boehner's tears as he fights against a woman's right to choose and privacy?  Taking that away and restricting it should be worthy of a least a handful of tissues.  Where are all the strict constructionists when it comes to a woman's health and privacy?

It is indeed a war.  This is not just a battle for the right to privacy.  This is a battle for equal rights.  This is a battle where we must demand that our rights are protected.  No more compromises with women's health as we had with the health care reform bill.  No more preventing women from gaining insurance for abortion.  Lest we forget, women's taxes go to the same IRS.

The 112th Congress is the most anti-choice ever.  The House will continue to speak of the smaller role of government while imposing government on women.  The battle lines are drawn; and the war will be lost unless women speak out against these laws and insist that our constitutional rights are threatened.  The fight is ours, we must work vigorously to defend our rights.  JAC is committed to this fight - are you?

If you read the propaganda produced by anti-choice advocates (and often repeated ad nauseam in the mainstream media), America's abortion crisis is that there is too much access to abortion. The reality, however, is that there is not even remotely enough access to abortion: 87% of counties in the United States have no abortion services. (That number increases to 93% if you remove urban counties.) There are a lot of reasons for this; lack of demand is not one of them.
Cleverly, Planned Parenthood of the Heartland in Iowa found a solution. Since 93% of counties in the state have no abortion provider, they created a service based on successful models of health care delivery in rural areas. Sarah at Feminists for Choice explains how it works:
The woman still has an ultrasound to determine fetal age; is examined by a nurse; and undergoes counseling. The doctor still reviews the woman’s chart and talks with her before prescribing the pills. Again, once all the information is reviewed, the doctor can choose to prescribe RU-486, known as the "abortion pill." The only difference is that the doctor is not in the room. He/she is teleconferencing with the patient. The woman receives the medication from a locked drawer opened remotely by the doctor, takes her first dose at the clinic, then follows up with a second dose at home. She comes back to the clinic for follow up exams and care. Nothing about the process is different than if the doctor is in the room, except that the doctor doesn't need to travel hours and hours to get to the clinic and there are no protesters outside threatening to shoot him/her and his/her family.
Of course, the people who will do anything to ensure that everyone must live by their religious beliefs about abortion tried to stop this. Of course, they claimed it is to safeguard women's health. (As Robin Marty at RH Reality Check wrote back in March, antis noted that RU-486 has caused 29 deaths. For goodness sake's then, I say we should ban pregnancy immediately! Over 700 women in the US die each year from problems related to pregnancy or childbirth which is terrifying and sobering, and thus if you care about women's health [1.7 million women have health complications due to pregnancy every year], the obvious answer is to stop pregnancy now! Sigh.) Maybe they think it is safer to have an illegal abortion. I'm not sure. Whatever the case, they filed complaints that it violated state laws mandating that doctors perform procedures. But that's exactly what is happening with telemed prescriptions since the doctor would only give her the pills anyway. (Again, this is not a surgical procedure in any way, shape, or form.) Fortunately, as reported by Jezebel, the medical facts prevailed and the 19 clinics across the state can continue the practice.
From what I understand based on a casual conversation I had on Monday night, Planned Parenthood is looking to expand this great model. Hence the antis are gearing up to have them banned in their state. (Oh Nebraska! Really?) So it goes: One step forward, two slaps in the face, and a poignant reminder that Roe v. Wade is meaningless if there's no one in your community to offer a safe abortion when you need one.

The abortion rate has remained steady in the US between 2005 and 2008 after declining for a decade. Researchers suggest that the recession as a possible factor. Incidents of harassment at the offices of nonhospital abortion providers increased in the same time period.
The abortion rate was at its highest recorded in 1981. The study, which was conducted by the Guttmacher Institute, is to appear in the March 2010 issue of Perspectives on Sexual and Reproductive Health.
Researchers also found that early medication abortion using the abortion drug RU-486 (mifepristone) has become an integral part of abortion care. 59% of all known abortion providers now offering this as an option. Indeed, the proportion of nonhospital abortions using early medication procedures increased from 14% to 17%. In addition, the number of abortion providers changed little from 2005 to 2008, from 1,787 to 1,793. 87% of counties in the US continue to have no abortion providers; 35% of women of childbearing age live in those counties.
However, the study reported a 'disturbing increase' in the proportion of large nonhospital providers experiencing antiabortion harassment, from 82% in 2000 to 89% in 2008. From a press release from the Guttmacher Institute:
 
Harassment was particularly common among providers of all sizes in the Midwest and South. Picketing was the most common form of harassment (reported by 55% of providers), followed by picketing combined with blocking patient access to facilities (21%).
 
As to why the statistics on abortion have not changed, the January 11th Time magazine quotes Columbia University professor Beverly Winikoff:
Although there are no proven theories as to why abortion rates may not have continued their decline, Beverly Winikoff, a Columbia University professor and president of Gynuity Health Projects told USA Today that the recession may have played a role: "Don't dismiss the economy as a (strong) reason for not having a baby."
The January 11th Washington Post also cited the recession as a factor in the abortion rate plateauing. As it has become harder for women to afford contraceptives, there are more unplanned pregnancies and more women who 'decide they can't afford another child,' Rachel K. Jones, a senior research associate at the Guttmacher Institute, noted:
Previous recessions did not have a major impact on the trend in the abortion rate, but those economic downturns were not nearly as severe, Jones said.
Other observers agreed, saying women were making the painful economic calculus to skip a few routine bills to cover a $400 to $600 abortion rather than face supporting another child.
"We hear from women who are selling possessions and forgoing paying certain bills in order to afford the abortion care they need," said Vicki Saporta of the National Abortion Federation, which represents abortion providers. "Many of these women already have children and are struggling to care for the children they have and are very clear they can't afford to have another child."
The Washington Post quotes Randall K. O'Bannon of the National Right-to-Life Committee, who attributes 'increased efforts to promote abortions, especially medical abortions using the drug known as RU-486' as the reason for the abortion rate holding steady.
The Guttmacher Institute is a non-profit organization that seeks to advance sexual and reproductive health in the US and around the world through social science research, policy analysis and public education. For the study, "Abortion Incidence and Services in the United States, 2008," the institute drew on its 15th census of all known abortion providers in the United States for data; nearly 1800 providers were surveyed. Kampala — CATHOLICS forbid it, the African culture is against it, while the law in Uganda says it is murder. But this all pales in comparison to the Uganda Demographic and Health Survey (UDHS) 2006 report which says there are 297,000 abortions induced annually, while an estimated 65,000 women experience complications, but receive no treatment.Dr. Simon Musani, a gynaecologist/obstetrician consultant at MedAfrik Laboratory, Mbale, defines abortion as the termination of a pregnancy by the expulsion from the uterus of a foetus, resulting in death.
He says abortion can occur spontaneously due to pregnancy-related complications. "An abortion can be induced to preserve the health of the mother," he adds.
Musani says abortion can be induced using herbal concoctions, sharpened tools, physical trauma and other traditional methods.
"However, contemporary medicine utilises medication and surgical procedures to induce abortion," he says.
The 2006 UDHS report says one in four abortions are performed by doctors, four in 10 by nurses or midwives, and one-half by pharmacists or dispensers; two-thirds are done by traditional practitioners and seven in 10 of those which are self-induced result in complications. This includes excessive bleeding, fever, infection including pelvic inflammatory disease, increased risk of cervical cancer, damage to the uterus and high maternal mortality.
A UDHS study of women who were treated for pregnancy-related problems in three Kampala hospitals, found that 21% of maternal deaths were due to post-abortion complications.The main reason behind the complications is lack of access to post-abortion care services and accurate family planning information.
Need for post-abortion care
In Uganda, post-abortion care (PAC) is permitted under the Ministry of Health's reproductive health policy.
"PAC is the emergency care provided to women following pregnancy loss," says Dr. Kari Koita of Mulago Hospital.
Post-abortion care also serves to promote safe abortion to the extent allowed by law - if the mother's life is in danger.
Services provided
Dr. Charles Kiggundu, an abortion focal person at the Ministry of Health and Mulago Hospital, says in Uganda, post-abortion care services include emergency care, where the mother is treated for infections or diseases suffered and other complications.
"We induce or remove the unwanted parts of a foetus that have remained in the womb using manual or eleDr. Frank Kaharuza, the president of the Association of Obstetrics and Gynaecology of Uganda, says almost all hospitals and health centres that treat abortion complications can provide antibiotics for women with infections, and also prescribe oxytocics - drugs that stimulate contractions of the uterus - to complete the expulsion of the foetus.
He explains that health facilities are also equipped with drugs to stop bleeding of mothers after an emergency, adding that Oxytocin and Ergometrine are the drugs used for this.
Kaharuza says family planning services are also given to advise the couple to delay the next pregnancy.
"The woman is also counselled to relieve her and the family of trauma. This is because they experience problems like alcoholism, divorce or domestic violence after abortion," Kaharuza explains.ctric vacuum aspiration."Kiggundu adds that they also screen for and treat sexually transmitted infections and HIV/AIDS.Constraints
Kaharuza, however, explains that Oxytocin and Ergometrine are unstable at room temperature and administration is difficult for untrained health workers. They are in liquid form and administered as an injection, he adds.
He also regrets that PAC services remain limited to a few urban settings, largely due to low level of investment in infrastructure, equipment and technical skills to undertake it.
"Although nearly 70% of PAC is provided by health centres or private midwives, skilled and complex post-abortion care is largely available only in hospitals," Kaharuza says.
Kiggundu says in Uganda, post-abortion care services are free in government health facilities.
A comprehensive package costs sh50,000 in private not-for-profit hospitals and between sh40,000 to sh140,000 in private for-profit health centres.Misoprostol could save women
Kiggundu says Misoprostol has been noted to greatly help in the prevention and management of post-partum haemorrhage. It is also used for treatment of incomplete/missed miscarriages or induced abortion."Prof Florence Mirembe of the Gynaecology and Obstetrics department of Makerere University, says Misoprostol is cheap and easy to use.
"A dose may cost about sh5,000, and it can be administered by a trained health professional. But the dilemma is that it is not available in government health facilities," she says.
She adds that its only side-effects are diarrhoea, nausea, vomiting, chills and fever.
Kiggundu says in co-operation with the Ministry of Health, Mulago University Teaching Hospital and Mbarara University Referral Hospital have set up pilot projects to provide Misoprostol in their post-abortion care services.
"We are going to evaluate the findings after one year," Kiggundu says.
